Call the number on the back of your insurance card and ask about outpatient psychiatric benefits (CPT codes 99205 for an initial evaluation, 99214 + 90833 for follow-up visits). Ask about your copay, deductible status, and any prior authorization requirements.
Some plans require prior authorization for psychiatric services. If yours does, please let us know when scheduling and our office will work with your insurance to obtain the necessary authorization before your appointment.
We accept MinnesotaCare. For other Medicaid or Medical Assistance programs, please contact our office to verify coverage, as participation varies by managed care organization.
A superbill is a detailed receipt that includes the provider's NPI, diagnosis codes (ICD-10), procedure codes (CPT), date of service, and amount paid. You submit this to your insurer, and they reimburse you based on your out-of-network benefit level.
